If you would like to file a complaint against a funeral establishment, embalmer and/or funeral director, please review Complaint Procedures before submitting a complaint.

Fill Out A Complaint Form

We accept typed and hand-written complaints if they are legible. All complaints must be signed and dated. Embalmers & Funeral Director Complaint Forms must also be notarized.

If you have completed the downloadable complaint form, you may submit your complaint to our office by mail or email.

Please include copies of supporting documents. Do not mail originals.
